The agency now requires a warning about this withdrawal reaction in the prescribing information for both cetirizine and its chemical cousin levocetirizine (Xyzal). As far as we can tell, however, this alert is not included in the over-the-counter labels.

ถึงแม้ว่าทั้ง 3 โมเลกุลจะเป็น
Third Generation เหมือนกัน
แต่ก็ใช่ว่า จะไม่แตกต่างกันเลย
แล้วแต่คนชอบ... แต่เอาหลักพิจารณาไปก่อนว่า
จะเอายาตัวไหนเข้า รพ.ดี
เช่น ยาต้องอยู่ใน guideline
ซึ่งทั้ง3ตัว อยู่ใน ARIA guideline
หรือ โรงเรียนแพทย์ ยาเข้าไหม่ เป็นต้น
โดยทั่วไปแล้วหลักๆ ใช้อยู่ 2 indications
1. Allergic Rhinitis
2. Urticaria

มี Efficacy แตกต่างกันอย่างไรบ้าง

ในทุกวันนี้ ยาจะเข้า รพ. ลุงหมอ ต้องมานั่งอ่าน reviewยา แบบลงลึกทุกตัวก่อน ว่าใครจะได้เข้า เพราะผ.อ. สั่งคาถามาแต่ item เต็มตลอด คาถาเดียว ปวดหัวคนพิจารณา
การที่จะบอกว่ายา ตัวใดมี Efficacy ที่ดี
ต้องพิจารณาถึงองค์ประกอบ 3 ข้อดังต่อไปนี้
1. Receptor Occupancy
2. Onset of Action
3. Long Duration
Receptor Occupancy
หมายความว่า Histamine Receptor ถูกยาจับได้กี่%
ซึ่งสามารถคำนวณได้จาก drug affinity and free plasma concentration และค่าอื่นๆร่วมด้วย
ดังเช่นปรากฎในรูปภาพ
ซึ่งมีงานวิจัยทำเปรียบเทียบยาทั้ง 3 โมเลกุลนี้
ในชั่วโมงที่ 4 และ ชั่วโมงที่ 24 หลังจากให้ยาไป
พบว่า ชั่วโมงที่ 4 Fexofenadine มีค่า Receptor Occupancy สูงที่สุด อยู่ที่ 95% รองลงมาคือ Levocetirizine 90% และDesloratadine 71%
แต่เมื่อชั่วโมงที่ 24 พบว่า Levocetirizine มีค่า 60% รองลงมาคือ Desloratadine 43% และ Fexofenadine 12%

แน่นอนครับว่า หากมีอาการผื่นแพ้ คัน
หรือ อาการน้ำมูกไหลมาก
ผู้ป่วยส่วนใหญ่มักชอบบอกว่า
หมอขอยาดีๆนะ เอาแบบหายไวๆ
เรื่องนี้ก็สำคัญไม่น้อย
มีงานวิจัย ที่ได้รวมรวบข้อมูล เหล่านี้มาให้ ซึ่งก็พบว่า
ทั้ง 3 โมเลกุลนี้มี Onset of Action ที่เร็ว
โดย Levocetirizine นั่น ออกฤทธิ์ได้ภายใน 0.5 ชั่วโมง ในขณะ Fexofenadine และ Desloratadine ออกฤทธิ์ 1-2 ชั่วโมง และ 1-3 ชั่วโมงตามลำดับ
นอกจากผู้ป่วยจะชอบยาที่หายเร็วๆแล้ว
บางทีก็ชอบบอกคุณหมอ อีกว่า
ขอกินยาวันละครั้งเดียว ก็พอนะหมอ
ซึ่งจะรับประมานวันละครั้งได้หรือไม่นั่น
อาจจะต้องพิจารณาถึงเรื่อง Long Duration of Action
ซึ่งจากงานวิจัยเดียวกันนั่น พบว่า
Levocetirizine สามารถออกฤทธิ์ได้ มากกว่า 24 ชั่วโมง ในขณะที่ Desloratadine และ Fexofenadine ออกฤทธิ์ได้ 24 ชั่วโมง
และข้อที่ว่าคนไข้เป็นไง
ต้องระวังในคนที่มีปัญหาเรื่องตับและไตไหม ก็ต้องมาพิจารณาร่วม

Treating Allergies
Medications
Antihistamines
Common medications such as Zyrtec (cetirizine), Allegra (fexofenadine), Claritin (loratadine), and Xyzal (levocetirizine) are safe to use on a daily basis. Loratadine and cetirizine are considered the antihistamines of choice for women who are pregnant.
Nasal sprays
Intranasal steroid sprays are safe to use on a daily basis to help reduce inflammation in the nose. Some nasal sprays, such as Flonase (fluticasone) and Nasacort (triamcinolone), are available over the counter.
Decongestants
Decongestant nasal sprays are not recommended for daily use because of side effects.
Eyedrops
Eye allergies can cause red, itchy, watery, and swollen eyes. Over the counter eyedrops, such as Zaditor (ketotifen) and Pataday (olopatadine), can help relieve symptoms.
A fixed-dose combination of montelukast plus levocetirizine was safe and effective for the treatment of perennial allergic rhinitis and asthma compared with montelukast alone.
In acute urticaria, adding prednisone to levocetirizine offers no additional benefit to patients compared to levocetirizine alone, according to a new study published in Annals of Emergency Medicine.
Patients received either levocetirizine 5mg daily for 5 days and prednisone 40mg for 4 days (n=50) or levocetirizine 5mg daily plus placebo (n=50). The primary endpoint was itching relief two days after the emergency department visit.
Results showed that the placebo group actually had a greater reduction in itch score, with 76% of this group having an itch score of 0 compared to 62% of patients in the prednisone group (95% CI; –31% to 4%). Additionally, more patients in the prednisone group reported relapses than the placebo group, at 30% versus 24%, respectively (95% CI; –23% to 11%).
